Below is the Good Health Card (GHC) sliding fee scale for services* beginning January 1, 2025.
*Fees for commercial insurance holders and self-pay patients will differ, consistent with HRSA health center program and insurance plan requirements.

GHC = Good Health Card
1 Includes basic radiology, procedures, and integrated behavioral health services related to a medical visit.
2 CT-only (other radiology services included in medical fee.)
3 Restorative and Speciaty flat fees for GHC B, C and D patients are applied each time the associated CDT code is used. Specialty nominal fees for GHC A patients are applied each time the associated CDT code is used.

You can view all of your billing information through MyChart. If you have any questions or want to learn about our payment options, call us at 254-313-4450. As a reminder, you will not be denied service regardless of your ability to pay.
