Whether you need a copy of your records, want to transfer them to another provider, or have questions about your health history, our team is ready to assist you. View Medical Records Online with MyChart
MyChart gives you secure online access to parts of your medical record. With MyChart, you can:
New patients can sign up for MyChart with an activation code provided in the enrollment letter or after visit summary.
Request a Copy of Your Medical Records
If you need an official copy of your medical records for yourself, another provider, your insurance, or another purpose, please complete a Medical Records Request Form.

Please complete all required fields on the form so we can:
- Confirm your identity
- Understand which records you need
- Know where and how to send your records
*Be sure to sign and date the form. Unsigned forms cannot be processed.
3 Send your completed form
You can mail or fax your completed form to
