Our program provides discounts for medical, dental, and pharmaceutical services.

Title V Program
Our Maternal and Child Health (MCH) Program can assist you in applying for state programs such as CHIP, Title V and the Family Planning Program, which cover services for pregnant women and children.

1 Gather Documents for Verification
Proof of:
- Identification
Valid photo ID, birth certificate, or Social Security card - Address*
Current utility bill or lease agreement - Monthly Household Income*
Recent check stubs, letter from employer, self‑employment form, child support,
disability, retirement, or Social Security Income - Medical Benefits
Medicaid, Medicare, commercial insurance card, county card, etc. - Household Member Information
IDs, Social Security cards, or birth certificates
*Proof of income and address must be dated within the last 30 days.

FAQ
The household consists of a person living alone or a group of two or more persons related by birth or marriage including common law, or adoption, who reside together and are legally responsible for the support of the other person.
- For children under 18 years of age you must provide a birth certificate or social security card.
- Children 18 and older will be counted as a separate household
- You must provide custody documents for any child in your care at the time you apply
- You must provide Guardianship or Power of Attorney documents for any adult in your care
Our Patient Access Center can help, for new patients, please call 254-313-4610 and existing patients call our Billing department at 254-313-4450.
Even if you have health insurance, Waco Family Medicine offers a discount program to individuals and families with annual incomes at or below 185% of current HHS federal poverty guidelines. Eligibility for this program is based solely on family size and income. Significant discounts may be available for services received at Waco Family Medicine. Contact our Patient Access Center for more information.
