Below is the Good Health Card (GHC) sliding fee scale for services* beginning January 1, 2025.
*Fees for commercial insurance holders and self-pay patients will differ, consistent with HRSA health center program and insurance plan requirements.

GHC = Good Health Card
$## = Eligible for Prompt Pay Discount
1 Includes basic radiology, procedures, and integrated behavioral health services related to a medical visit.
2 CT-only (other radiology services included in medical fee.)
3 Restorative and Speciaty flat fees for GHC B, C and D patients are applied each time the associated CDT code is used. Specialty nominal fees for GHC A patients are applied each time the associated CDT code is used.

Waco Family Medicine offers significant savings for patients with the Good Health Card who pay promptly—a great way to reduce your costs while receiving high-quality care. Prompt Pay Discounts for Good Health Card
- Dental Services: Receive a 10% discount when you pay promptly.
- Medical, Radiology and Behavioral Health Services: Enjoy a 50% discount for prompt payment (excludes nominal fees or GHC A.)
How It Works
- Discounts are applied when payments are made before or at the time of your visit.
- Eligible for all patients unless limited by certain third-party agreements (such as the terms of your insurance plan.)
Additional Details
- Discounts are not available for nominal fees.
- Payments must be made with cash or credit card—personal checks are not accepted.
- All prompt pay discounts are handled through our Billing Department to ensure accuracy.
