I want to announce, to you and yours, that I am leaving Waco Family Medicine (WFM), and listing a date of April 07, 2024.

Other clinicians in the WFM system are available to care for you after my departure. Your medical records will remain on file at WFM and are readily available to any of the clinicians that may assume your care hereafter.

If you desire to see a clinician outside of the WFM system, please complete a release of information form at that new clinic, and we will provide them with a copy of your records. You may also directly request a copy of your records from Waco Family Medicine.
