Through assistance programs like Medicaid, CHIP, Good Health Card and Title V, we will work with you to see what you and your household may qualify for to use at Waco Family Medicine clinics and pharmacy.

Prescription Discounts: Prescriptions must be written by a Waco Family Medicine doctor and must be picked up that the Waco Family Medicine Pharmacy to get a discount. You may be eligible to receive anywhere from a 50% to 80% discount.

About Our Programs
Even if you have health insurance, you may be eligible for the Good Health Card program. The Eligibility Department at Waco Family Medicine is available to help our patients apply for financial assistance.

Good Health Card
Waco Family Medicine offers this discount program to individuals and families with annual incomes at or below 200 % of current HHSC federal poverty guidelines. Eligibility for this program is based solely on family size and income. Discounts of 50% or more may be available for services received at Waco Family Medicine. Contact our eligibility department for more information.
Family Planning Program
This is a state program that helps patients seeking financial assistance with Family Planning/contraceptives services.
Title V Program
Maternal and Child Health program assists patients with annual incomes at or below 185% of current HHSC federal poverty guidelines who are not eligible for Medicaid/ CHIP, CHIP Perinatal or another coverage and offers preventive / primary care services for pregnant women and children. Contact our eligibility department for more information.

At the time of your evaluation you will also be screened for the Good Health Card Discount Program. This will help with prescriptions and medical visits other than prenatal as well as fill out a CHIP Perinatal application. The Good Health Card will be issued for applicants under 21 years of age to help discount medications only.
In order to apply for any program Waco Family Medicine offers, you must provide the following documents for all household members. Please be advised that proof of income and address must be from the last 30 days.
- Proof of identification for all household members: Any valid photo ID, birth certificate, or social security card
- Proof of household monthly income: Check stubs, letter from employer, self-employment form, unearned income such as child support, Social Security Income, disability, retirement etc.
- Proof of address: utility bill or lease
- Proof of medical benefits: Medicaid, Medicare, commercial insurance card, county card etc.
- Household members information: ID’s, social security cards, birth certificates

The household consists of a person living alone or a group of two or more persons related by birth marriage including common law, or adoption, who reside together and are legally responsible for the support of the other person.
- For children under 18 years of age you must provide birth certificate or social security card.
- Children 18 and older will be counted as a separate household
- You must provide custody documents for any child in your care at the time you apply
- You must provide Guardianship or Power of Attorney documents for any adult in your care
Billing Department
The Billing Department understands that health insurance and bills can be confusing. The Waco Family Medicine Billing Department can help you understand your financial account. If you are unable to pay the full balance on your account we recommend you get in touch with our Eligibility Department. It is part of our commitment to provide financial advocacy for our patients.
