Transfer Request

At Waco Family Medicine we want to make sure you are getting the best care you deserve. If you need make a change to your WFM clinic or clinician, please consider the following before submitting a request.

• You must be a current Waco Family Medicine patient.
• You must have seen your WFM clinician within the past 3 years.
• This request is not for patients who want to transfer from a different healthcare system to Waco Family Medicine.
• This request is not a guarantee that you will be transferred to a new clinician.
• All requests will be reviewed before a final decision is made.
• You will be notified by phone or mail about the result of your transfer request.
• Patients are allowed only one transfer request, so please do so wisely.
